Actor handbook?

I was just wondering how many of you use a handbook for your actors and volunteers. I was thinking of using one for my team. Can anyone supply maybe a template of how one is formatted? I want to include all safety precautions and other rules that the actors are expected to follow. Thanks.

I think someone was selling a premade book you could buy and make copies. I dont remember who it was though. I would like to start doing this also. So far I have given a verbal class before the haunt opens and I usually forget stuff.
